Adventure Wellbeing School ("the school") is the data controller for the information collected through this form.
Your child's name, date of birth and class; the consent choices you make; any medical conditions, allergies, dietary or care information you provide; emergency contact details; and your name, your relationship to the child and the date you sign.
To obtain and record parental consent for school activities, trips, medical care and support services, and to help keep your child safe. Our lawful basis is a task carried out in the public interest (UK GDPR Article 6(1)(e)). Where you give us health information, we rely on your explicit consent and the provision of health and social care (Article 9(2)(a) and (h)), and on safeguarding where relevant.
Securely in Microsoft Azure data centres in the United Kingdom. It is encrypted both in transit and at rest, and access is restricted to authorised school staff signing in with a school account.
Only staff and professionals directly involved in your child's education, care or safety — for example a therapist you have consented to. We do not sell your information or use it for marketing.
We keep this information until 12 months after your child leaves the school, after which it is securely deleted.
You can ask to see the information we hold, ask us to correct it, or withdraw your consent at any time by contacting the school office. You can also ask us to delete it, subject to our legal obligations.
